To obtain a rebate from Medicare please bring your referral from your General Practitioner (GP) to Dr Myran Ponnam-Palam.
Your GP may give you a 12 month referral however if you have received a referral from a Specialist this will only be valid for 3 months unless specified otherwise.
If you would like a fertility appointment with Dr. Myran both you and your partner will need to appear on the referral. Please bring this to your first appointment.
2. Your Medicare details
(and Private Health Insurance Details if applicable)
At your first appointment we will ask you to complete a registration form including your contact details, Medicare number (and Private Health Insurance information if applicable). This information will help us to submit claims to Medicare on your behalf or alternatively provide you with receipts to submit to your private health insurer if you are a non- resident.
3. Any results and list of medications
Please bring your (and/or your partners) latest:
- Blood test results
- Operation reports or;
- Semen analysis results
- Medications – List of the medication and dosages that you are currently taking (fertility and non-fertility related)
4. Your medical Questionnaire
In order to give Dr. Myran the best chance of helping you conceive could you and your partner (if applicable) please complete the attached medical questionnaire. It’s designed to provide us with a more complete history of your journey to grow your family. This in conjunction with any blood test and ultrasound results will assist Dr Myran in having a more complete understanding of your medical history and a more valuable initial discussion regarding your fertility treatment options.
Having a baby revolves heavily around the Menstrual Cycle
Fertility treatment is based around the female’s menstrual cycle. It’s helpful if you keep a menstrual chart and notes of when:
- Your menstrual cycle starts
- How long it lasts and;
- If and when you have any pains or spotting between menstrual cycles.
The fertile window
The most effective time to conceive is during your fertile window (about six days every month). These six days occur for most women during the five days leading up to, and the day of, ovulation, when your body releases an egg.
The fertile window usually starts about 5 days after your period (bleeding) has finished.
